CareCredit, a subsidiary of GE Capital Retail Bank, has had to refund up to $34.1 million to possibly more than 1 million people for persuading them to sign up for credit cards they thought were interest-free — but actually charged high rates, according to the Consumer Bureau. In Dollars and Dentists, FRONTLINE and the Center for Public Integrity found that CareCredit and other similar companies are part of the core business model used by Aspen Dental and hundreds of other dental and health care providers. Providers are paid by credit card companies upfront for a patient’s entire treatment plan when they sign the dotted line, so providers have an incentive to encourage consumers to consider more expensive treatments.
CareCredit is one of the largest of these companies, and is offered by about 175,000 dental and healthcare providers nationwide as a payment option, according to the Consumer Financial Protection Bureau. Most consumers are provided with a deferred interest plan that earns 26.99 percent annual interest; if the balance is not paid in full by the end of a certain period of time, the consumer becomes interest dependent.

But that’s not what consumers are being told, the bureau’s investigation found. Since January 2009, dental and healthcare providers have misled consumers about deferred interest, leading them to believe it was an interest-free card, the bureau said. Even some of the office staff members tasked with explaining the deal said they were confused about how it would work. And consumers aren’t always provided with a copy of the CareCredit agreement for review.
The bureau found that CareCredit lacked adequate oversight of the registration process and offered little or no training to service providers so they could adequately explain the agreement.
“Medical debt is already a huge problem for many Americans,” Richard Cordray, the bureau’s director, told reporters. “Poor credit card transparency shouldn’t make matters worse.”
The bureau ordered GE Capital to set up a $34.1 million fund and notify 1.2 million consumers that they could apply for reimbursement. They must also increase disclosure for consumers, including requiring CareCredit representatives to complete transactions processing more than $1,000, and conducting training for service providers who offer their products. The company has cooperated with the bureau.

By marketing to people who haven’t seen a dentist in years, Aspen Dental often provides new patients with treatment plans that cost thousands of dollars. Aspen Dental denies that its dentists have a stronger financial incentive than other dentists or that their bonuses affect treatment. Fontana, founder and chief executive of Aspen Dental, based in East Syracuse, NY, says dentists won’t perform unnecessary treatment because “it’s just not in their DNA.”

“I’m not even sure what corporate dentistry means, because we have no influence over dentistry,” says Fontana.
He said Aspen Dental freed dentists to focus solely on the patient, as the company handled back-office tasks like marketing, accounting and billing. All but four states prohibit anyone who is not a dentist from owning a practice on the assumption that dentists are trained and motivated to put patients before profits.
But Senator Charles Grassley, a Republican from Iowa, questioned whether dentists on the corporate dental chain were free from corporate pressures to maximize profits. Grassley, a ranking member of the Senate Finance Committee, wouldn’t talk about Aspen Dental specifically, but he did have committee investigators probing the company and other private equity owned chains for months.
“Because when private equity firms get involved,” explains Grassley, “you have to understand that their motivation is to make money. And they are not dentists. And dentists have to determine… what is good for teeth… Not private equity managers on Wall Street.”
