Over the past 80 years, the effects of the Mediterranean diet on overall health have been a topic of constant interest to medical and scientific researchers. Along with the global increase in type 2 diabetes, many studies conducted over the past 20 years have shown the benefits of a Mediterranean lifestyle for people with or at risk of developing type 2 diabetes. However, despite the abundance of evidence, there are concerns among scientists about the reliability of data related to this topic. This review suggests the beginnings of the Mediterranean diet and its implications for the prevention and treatment of type 2 diabetes. The number of people with type 2 diabetes is constantly increasing, especially in the Balkan Peninsula and North Macedonia. Given that North Macedonia, as well as the majority of the Balkan countries, is low- to middle-income, there is a need for a specific affordable dietary pattern to reduce the increase in diabetes and improve glycemic control. We reviewed the available literature on the Mediterranean diet and its effects on glycemic control, lipid profile, and metabolic outcomes in people with or at risk of developing type 2 diabetes.

It is now known that people with diabetes are prone to chronic micro- and macrovascular complications that affect their quality of life. In addition, people with diabetes have a reduced life expectancy [1] and a doubled risk of cardiovascular disease and cognitive impairment [2, 3, 4]. Previous studies have also shown that people with diabetes are significantly more likely to develop certain types of cancer [1, 5, 6].

Treatment for type 2 diabetes has improved in quality and quantity over the past two decades. Pharmacological methods have been particularly improved; Therefore, doctors choose the appropriate drugs differently. As maintained by the American Diabetes Association (ADA) 2020 diabetes guidelines, the treatment of type 2 diabetes includes specific criteria related to the expected efficacy of therapy, potential side effects, cost, and impact on body weight. factors such as cardiovascular risk, microvascular complications and patient preferences [7]. In recent years, glucagon-like peptide 1 (GLP-1) receptor agonists, sodium-glucose co-transporter-2 (SGLT2) inhibitors, thiazolidinediones, and dipeptidyl peptidase 4 (DPP-4) inhibitors have increased dramatically. Nevertheless, despite this significant pharmacological improvement, the Diabetes 2020 standards of care by the ADA include a separate chapter focusing on lifestyle changes that improve diabetes and, therefore, overall health [7, 8, 9]. The Finnish Diabetes Prevention Study (DPS) was one of the first controlled, randomized studies to demonstrate that type 2 diabetes can be prevented by lifestyle interventions [7]. Diabetes was found to be reduced by 58% compared to the control group [10]. In addition, these findings were supported by the Diabetes Prevention Program (DPP), which showed a similar risk reduction in addition to superior outcomes compared to metformin treatment [ 11 ]. These studies have shown that type 2 diabetes can be prevented and treated through behavior change and modification. While lifestyle modification remains the most beneficial and theoretically the easiest recommendation at this time, making healthy lifestyle and nutritional choices can be difficult for people with type 2 diabetes.

The International Diabetes Federation (IDF) classifies people who are overweight or obese, have a poor diet, a sedentary lifestyle, and/or a family history of diabetes as having a high risk of developing type 2 diabetes.
