A new study conducted by Stanford icine researchers compared two popular low-carbohydrate diets – ketogenic and iterranean – in terms of their impact on blood glucose levels, cardiometabolic risk factors, weight loss and nutrition, as well as the ease with which people can adhere to them.

The ketogenic diet is a very low carb, very high fat diet that involves drastically reducing carbohydrate intake. The iterranean diet is a low-carb, moderately high-fat diet that emphasizes vegetables, legumes, fruits, whole grains, olive oil, and fish.

Both diets received high marks for controlling blood glucose levels and aiding in weight loss, but the ketogenic diet was lower in several nutrients, especially fiber, and harder for study participants to stick to long term, according to findings published May 31 in the American Journal. of Clinical Nutrition.

A very low carb ketogenic diet meets these criteria. But its dramatic rise in popularity in recent years has been troubling nutritionists like Gardner.

“The less carbs you eat, the more you eliminate whole food groups that are considered very nutrient-dense and healthy,” he said, noting that to achieve extreme carb restriction, the ketogenic diet forbids legumes, fruits, and whole grains. “What is it about this diet that would be so convincing that you would give up some of these major principles of health and nutrition?”

In a new study, conducted from June 2019 to December 2020, Gardner and his team recruited 40 adults with type 2 diabetes or prediabetes to try both the ketogenic diet and the Iterranean diet. Half of the participants started with the ketogenic diet and the other half with the iterranean diet. After 12 weeks, the groups switched and tried a different diet for 12 weeks. This crossover design allowed participants to act as their own controls.

In the ketogenic diet phase, participants used a version known as a well-formulated ketogenic diet. They were advised to limit carbohydrates to 20-50 grams per day (about 80% less than usual), proteins to 1.5 grams per kilogram of ideal body weight per day, and to consume fats as much as they wanted. They were asked to eat at least three servings of non-starchy vegetables a day.

In the Iterranean diet phase, participants were advised to follow a mostly plant-based diet that included vegetables, legumes, fruits, whole grains, nuts and seeds, as well as fish for animal protein and olive oil for fat.

Both diets encouraged eating plenty of vegetables and eliminating added sugars and refined grains as much as possible. The main question, Gardner said, was whether the keto diet provided the added health benefits of eliminating legumes, fruits, and whole grains.

Keto Diet And Diabetes Risk

To ensure participants’ success, the study sent ready-to-eat food to both groups during the first four weeks of each diet via food delivery service methodology. Gardner said that aside from feeding people all their meals in the lab, this approach was the best way to maximize diet adherence.

For the remaining eight weeks, participants were responsible for choosing and preparing their own food, giving the researchers insight into how people realistically adhere to the diet. Participants were encouraged to be honest about how well they were following the diet. “Don’t think you have to impress us,” they were told.

At various points, the researchers took blood samples to assess blood glucose control, lipid control, body weight, and other clinical measurements. Throughout the study, participants were also asked about their adherence to and satisfaction with their diet. Thirty-three participants completed the study and several dropped out due to COVID-19 disruptions.

The researchers found that both diets improved blood glucose control, as indicated by similar decreases in HbA1c levels (9% on keto and 7% on iterranean). Weight loss was also similar (8% on keto and 7% on iterranean), as were improvements in insulin and fasting glucose, HDL cholesterol, and the liver enzyme ALT.

Reverse Type 2 Diabetes With The Keto Diet

Each diet had another statistically significant benefit: LDL cholesterol levels rose on the keto diet and fell on the iterranean diet – a point for the iterraneans. Triglycerides decreased on both diets but fell more on the keto diet – point for the keto diet.

In terms of nutrients, the ketogenic diet provided less fiber; thiamine; vitamins B6, C, D and E; and phosphorus. Only vitamin B12 was higher on the ketogenic diet.

As expected, after the food was provided, the participants adhered relatively well to both diets, scoring an average of 7.5 on a 10-point scale of adherence. When participants had to provide their own food, adherence to both diets dropped by about two points on average.

“The one thing they all did quite well was limiting added sugar and refined grains. That was the main message of both diets,” said Gardner.

Keto And Mediterranean Diets Both Help Manage Diabetes, But One Is Easier To Maintain

“The keto diet was more polarizing,” he said. “We heard some people just couldn’t do keto because it was too restrictive.” However, according to Gardner, some people found it easier, at least initially, to focus solely on avoiding carbohydrates.

When the research team met with participants three months after the study, on average, they maintained lower blood glucose levels and weight loss. Notably, they ate closer to the Iterranean diet than the keto diet. Even participants who followed the keto diet almost perfectly during the trial largely dropped out afterwards.

Gardner said eliminating legumes, fruits, and whole grains to achieve a very low-carb diet had no additional overall health benefits. For those with diabetes or prediabetes, the less restrictive iterranean diet was similarly effective in controlling glucose and possibly more balanced.

“The focus should be on limiting added sugars and refined grains and emphasizing the inclusion of vegetables,” said Gardner. “There’s no reason to limit heart-healthy, high-quality carbohydrate foods above all else.”
