Please include your subscriber ID and display address on your application to help us confirm your copay/member price. Please note that if you apply for a Surest plan with UnitedHealthcare, your application will be rejected.

To ensure pricing reflects the best and most recent data, member copays/prices are updated annually. Accordingly, you should always check the current copay/membership price before charging for a service. While co-pay/participant price may change, your negotiated rate for the service will not change.

For members with Surest Flex plans, a small set of scheduled treatments and treatments require the member to activate coverage at least 3 business days in advance or they will not be covered for the treatment. You must confirm the member’s eligibility and benefits, and that the member has activated coverage for the service in advance for certain treatments or procedures.

The Prior Authorization/Notice requirements set out in the UnitedHealthcare Administrative Manual apply to Surest Flex plans. These are services for which the member must activate coverage for certain scheduled treatments or treatments at least 3 business days in advance.
